Our mission is to ensure no one recovers alone. Post Care Partners bridges the gap between hospital discharge and independent living, providing compassionate, meaningful support during the most vulnerable days of the healing journey.

The Problem We Solve
Post Care Partners was born from a personal experience watching a family member navigate recovery alone after surgery. We saw firsthand how the gap between hospital discharge and independent living leaves people vulnerable, and we decided to do something about it.
Every year, millions of Americans have surgery and go home the same day or shortly after. Hospitals expect someone to help with recovery, but many patients do not have that support.
For adults who live alone, are without reliable support nearby, or simply need an extra hand, the days after surgery or discharge can feel uncertain and hard to manage.

What We Do
Post Care Partners provides professional, non-medical recovery support for patients transitioning from hospital to home. We are not a home health agency or a medical provider.
Our care partners handle practical daily needs that medical services often do not cover, including transportation, meals, medication reminders, companionship, errands, and home preparation.
We also provide support with activities of daily living — bathing, dressing, and mobility — so clients can move safely and maintain their dignity throughout recovery.
We coordinate with your medical team, keep family informed when requested, and help you stay on track during recovery.

Who We Serve
We serve Scottsdale and the surrounding communities, supporting patients who:
- Live alone or without nearby support
- Are having outpatient or scheduled surgery
- Are being discharged after a hospitalization
- Want professional support during recovery
- Need transportation to and from surgical procedures
- Prefer to recover at home rather than in a facility
- Have family or friends who aren't available to help

Meet the Founder
Why I Started Post Care Partners
Post Care Partners was born from a moment of fear and a flash of clarity. A few years ago, my sister underwent open-heart surgery at Stanford Health. She was 29 years old. After months of being dismissed by doctors who told her it was anxiety, she had finally fought her way to the right diagnosis, the right specialist, and the operating table.
Our family rallied around her. We rotated shifts, rearranged our lives, and showed up. We were lucky, we had the flexibility to do that.
But about a week after surgery, her husband had to return to work. My family had gone home. And my sister, one week out from open-heart surgery, unable to lift more than five pounds, barely able to get out of bed, was alone.
We flew back out immediately.
But sitting with her that week, one question kept running through my mind:
What happens to the people who go home to an empty house? The ones whose families can’t fly out, whose partners have no leave left, those who have no one to call?
I looked for an answer. I couldn’t find a reliable one. So I built it.
Post Care Partners exists for every person who deserves to have someone there, during the hardest, most vulnerable days of recovery, but doesn’t have that person. We show up so no one has to recover alone.
